Pathway to Independence is a collaborative programme designed to support individuals transitioning from secure care into independent community life. The programme provides a structured, inclusive, and transformative approach, equipping service users with essential life skills, personal confidence, and wellbeing strategies.
By partnering with trusted voluntary sector organisations, Pathway to Independence delivers tailored educational, recreational, and employment-focused activities to help individuals build resilience, social connections, and long-term stability.
The initiative aligns with Birmingham & Solihull Mental Health NHS Foundation Trust's commitment to tackling health inequalities, promoting equality, and enhancing patient outcomes through community-based engagement.
Pathway to Independence has been funded through REACH OUT, the West Midlands Provider Collaborative (PC) for adult secure mental health services and learning disability and autism services.
Catalyst 4 Change’s Role
Catalyst 4 Change is the lead organisation responsible for the management and overall delivery of the programme, including the Pathway to Independence Community Hubs and in-unit services within mental health facilities.
